Is there a specific form when changing address with county clerks offices?
Contact the clerk’s office and ask. Some counties do have particular forms. Some require a notary signature.
I have property in Stephens county too. When I moved they didn’t have a form. So I found one on Noble county and modeled one after it and they excepted it.
I created my own form and it has been accepted in multiple counties. I tried to attach to this reply but it’s not an option. Let me know if you would like it.
You can usually attach a document using the icon with the BAR and an UP ARROW.
Oklahoma now requires a 2" top margin and 1" side and bottom margins. Also return address. Keep in mind that in addition to your address, you should identify the legal descriptions in which you have an interest.

Here is the template I use when needed. It has been accepted from all counties that I’ve filed with in the past.
Keep in mind that county clerks require 2" top and 1" side and bottom margins. While there is some debate regarding whether this is required on all pages, it is bet to include on all pages.
