I’m attempting to put together an AFFIDAVIT OF ADDRESS CHANGE and want to be sure I add everything that should be included. Does anyone have a template by chance?

An affidavit to be filed to update your information as it relates to a mineral interest should have the followoing:

  1. Your current address
  2. Your name
  3. If your name has changed include your former name and the reason for the change, i.e. marriage, divorce or legal name change.
  4. The legal descriptions of the minerals.
