Can you request that the Division Order be written on a standard DO form?

I have learned that there are certain things that should be included in the Division Order that help protect your future and that there is a standard form

that includes them. The Division Orders I have received are bare bones and "Sign Here!"

Can I request that they use an industry standard form, and if so, What is the name of the form?

yes, the NADOA form. I will send it to you by email. Can't attach here very easily.

You can find it on the naro-us.org website under forms. However, you need to add the operator address to it. The new amended one left it off....
