I am the Personal Representative and only beneficiary for my deceased dad. His will is being probated in Morgan County where he had been living when he died. He has mineral deeds in Washington County and Yuma County How do I transfer them and get the form to do so?

You should receive this assistance from your probate attorney.
In many states an order of distribution is issued. Once filed with the county clerk it is treated as though the Personal Representative (executor, administrator) issued a deed. Some states follow an independent administration route which may require that the PR actually issue deeds. Check with your attorney to see how they work in Colorado.
